In India, a large rise in medical errors are reported mainly because of the miscommunication between the patient and the doctor regarding the various details of their health status and medical history.

1. No knowledge about the previous prescriptions
2. A new patient with no self knowledge about their medical records
3. No access to the details like allergic issues, genetic issues for the doctor
4. No centralised record storage for all hospitals
