I wish to share a project on Electronic Health Record management that I have been working on and wish to integrate the project with Ethereum blockchain network and git.
Medical Institutions all need electronic health record (EHR) systems. They need them to solve their record movement issues, to improve the quality and coherence of the care process, to automate guidelines and care pathways to assist clinical research, outcome management, and healthcare process improvement. EHRs are difficult to construct because the existing electronic data sources, e.g., laboratory systems, pharmacy systems, and physician dictation systems, reside on silos with varying structures, granularity, and code systems.
In our pilot trials, we connected with the radiology department at Fortis hospital, Noida (India) and found out that though they could store patient’s health and examination report using PACS but their concern on record management and utilization of patient’s radiology report to provide better and cost-effective treatment remain unsolved. Each hospital has a unique portal for entering into its database and different ways of navigation. Hence, it becomes very challenging when it comes to sharing or analyzing a person’s health report across different medical institutions.

Our Magic Solution
A tremendous number of imaging studies accompanied by radiological reports are accumulated and stored in hospitals' PACS (Picture Archiving and Communication System) in form of Electronic medical records. EHRs have potential to transform our medical system, clinical decision-making, and provide evidence of policies and support the hospitals in cases of litigation.
Using our solution, EHR as a service, we wish to create a platform which will help in legitimate storage of medical records, records movements etc. EHRs will help in data cleansing, creating EHR, records legibility and coordination with pharmacies, diagnostic clinics in case when movement of medical records is needed. The EHR application can be accessible using Internet browser, all the data will be on cloud and accessible everywhere and anywhere.

Primarily for the hospital and laboratories we wish to
provide the service of EHR file creation, centralized storage and record management. Further, this storage can be secured by linking with unique social number example in India personalized health report can be secured using Aadhar Number. These electronic records can also be moved from one hospital unit to another just in time with the analysis report.
Legibility of records, coordination with pharmacies, improved data collection. Electronic health records (EHRs) can eventually improve record keeping and coordination of medical care. They can make outcome studies — the best way to control medical costs – much easier and more reliable.

How it Works?
Using ORTHANC, an open source light weight DICOM server, and its REST API to query and retrieve patient’s data and radiology reports from a medical institution. The information retrieved will be cleansed and converted into DICOM image format. DICOM stands for Digital Imaging and Communications in Medicine. This file format is created to distribute and view medical images using a standardized file format.
EHR as a service will provide:
a. Creation of radiology files in DICOM format, examination reports and patient’s medical records from hospital visits. DICOM stands for Digital Imaging and Communications in Medicine. This file format is created to distribute and view medical images using a standardized file format.
b. Cleansing and validation of in-bound patient data available as medical images and examination reports.
c. EHR movement across different hospitals, laboratories, pharmacies on requirement basis using:
i) ORTHANC as an open source and light weight DICOM server for medical imaging, querying and retrieving patient’s data and radiology reports from a medical institution.
ii) An inbound web application for viewing medical examination reports and patient records. Complex applications can be built with ORTHANC Rest API
d. Authentication, authorization using unique SSN (social security number) for legitimately storing the medical records.
